4. A link to your blog or website where we can see more of your work

5. A professional picture of yourself

6. If you an a Poetry Author, please include:

-A picture of your most recent book

-A bio of the book

-The date it was published

-Where it can be published

When submitting your poem please be sure:

1. That your poem is 200 words or less

2. Title is included

3. Please keep it clean

4. Topic can be of your choice

5. Put Poet of the Month in the subject box (Or your submission will get lost)
